Understanding Congestive Heart Failure

Congestive Heart Failure (CHF) affects approximately 6.2 million adults in the United States. It is a chronic, progressive condition in which the heart muscle is unable to pump enough blood to meet the body's needs for blood and oxygen.

CHF is a leading cause of hospitalization among adults over 65, with high readmission rates within 30 days of discharge. Through proper monitoring, medication management, and lifestyle modifications, many of these readmissions can be prevented.
