About Provider Loop

Transforming cardiac care in skilled nursing facilities through AI-powered monitoring

Our Story

Founded in 2016 by Dr. Jeffrey Bander, Provider Loop was initially focused on reducing CHF readmissions. Recent advancements in AI have enabled the platform to expand its capabilities to continuous monitoring, dynamic care planning, and remote management, establishing it as a scalable, virtual cardiologist.

Since our founding, we've partnered with numerous NYC hospitals and skilled nursing facilities to improve cardiac care and reduce readmissions. Our innovative approach combines cutting-edge technology with clinical expertise to deliver exceptional care to patients in skilled nursing facilities.

Provider Loop has grown from a focused solution to a comprehensive platform that addresses the complex needs of cardiac patients in long-term care settings. Our commitment to improving patient outcomes and reducing healthcare costs drives everything we do.

Our Mission

Our mission is to transform cardiac care in SNFs through continuous, AI-powered monitoring, improving patient outcomes and lowering readmission rates.

We believe that every patient deserves access to high-quality cardiac care, regardless of their location or the resources available at their facility. By leveraging technology, we can extend specialized cardiac expertise to more patients and empower healthcare providers with the tools they need to deliver exceptional care.

Our Vision

To become the leading provider of AI-driven cardiac monitoring for SNFs, setting new standards in patient outcomes and healthcare cost reduction.

We envision a future where advanced cardiac monitoring is available to all skilled nursing facilities, where readmissions are dramatically reduced, and where patients receive proactive, personalized care that improves their quality of life. Through innovation and collaboration, we're working to make this vision a reality.

Our Leadership

Provider Loop is led by a team of experienced healthcare professionals and technology experts dedicated to transforming cardiac care.
